Position Title: Case Worker
Location: 337 Alexander Avenue, Bronx, NY 10454



Under the supervision of the Deputy Director of Social Services, the Case Worker, functioning as a collaborative member of the Social Services team, will provide services to participants referred to us from East Side House’s educational programs. In addition the Case Worker will work with the families of the students to address issues that are hindering student’s performance; the case worker will provide the case management services necessary to provide a satisfactory solution to the challenges and issues faced by the student and other family members.The following are some of the duties and responsibilities



Duties & Responsibilities:

• Conduct initial interview with individual to assess individual’s needs and strengths.

• Conduct home visits to assess socio-economic factors present in the family and neighborhood.

• In consultation with Program Director, develop an Individual Service Plan (ISP) for each participant, which will address program’s core areas.

• Implement the ISP working with participant to achieve measurable outcomes.

• Meet periodically with Program Director to review ISP to assess progress made or if goals have been met, or need to be modified.

• Provide referrals for participants that meet their immediate and long term needs. Follow up on referrals to determine outcomes and help resolve barriers.

• Use Strengths-based model through process in order to build strengths and work towards goals.

• Facilitate educational workshops and independent living skills.

• Conduct Educational and Employment Counseling

• Facilitate Group Counseling sessions

• Schedule and enroll participants in workshops for Family Budgeting and Consumer Education

• Under the supervision of the program Director, provide Individual and Family Counseling on a weekly, biweekly as determined for each participant

• Provide training on Mediation and Conflict resolution to participants

• Facilitate parenting skills training.

• Actively participate in team and program meetings. Provide suggestions for program improvement and implementation.

• Complete all program documentation and maintain confidentiality of all program files on participant and other family members. Using the Management Information System.

• Provide updates to Program Director on assigned participants.

• Participate in assigned staff development trainings.

• Provide other program support functions as required.



Education/Experience/Skills:

Bachelor in Social Work (BSW), or equivalent bachelor in the social services field, with three years of experience working as a case worker delivering comprehensive social services to youth ages 16 to 24. The individual should have excellent communication skills, both written and verbal, and a strong team orientation. Basic word processing skills required. Spanish is a plus.
